The Rejuvenation of the Mouth
The eyes play an important part in the image that we project of ourselves. After that come the mouth and the neck area. The choice of techniques for the rejuvenation of the mouth depends on the problem. Are lips thin and the mouth surrounded by deep wrinkles and/or folds? And what does the patient think? Is it the wrinkles that are the main problem, or is it the thin lips?
In young people, the best technique to augment thin lips is to use autologous material, such as fat, dermis fat (the deeper parts of the skin) or graft or fascia (a fibrous cover that we have on the outside of our muscles, e.g. in the temporal area or for example in scar tissue.)
If the patient is uncertain about the effect of these enlargements, a temporary filler, such as hyaluronic acid, can be used to begin with. Temporary fillers can however be an expensive alternative in the long run. Permanent fillers have been used by another well-known clinic and have led to very serious complications, in one case even the removal of the entire upper lip. Thus a serious warning against permanent fillers!
If the lips are wrinkled with grooves and folds, (not uncommon in our patients) a combination of filling and polishing surface treatment of the skin is best. This method gives a real rejuvenation of the skin as well as enlarged lips, which become attractive. This technique is very popular, even with somewhat older patients. If the lips are very thin and the mucosa cannot be seen from the front, it must be mobilized from the inside of the lips. We call this the V-Y- technique, and this is a somewhat larger operation.
Augmentation with autologous material is usually carried out in 1, 2 or 3 sessions. There is an advantage here: the patient can participate in the decision about the final result.
These procedures usually take place under local anaesthesia, sometimes in combination with intra-venous sedation in the arm.
Sometimes the corners of the mouth are pulled down (moue face or drooping mouth corners) which makes you look tired. This can be corrected by a face-lift, but sometimes we only need to perform a minor procedure called a corner lift.
